Summary :The Ambulatory Care Clinical Pharmacist practices as an integrated team member within a family medicine, internal medicine or specialized clinic. Following Comprehensive Medication Management principles, the ambulatory pharmacist collaboratively manages the medication related outcomes for chronic and complex disease states. Through referrals and practicing within the State of Wisconsin pharmacy regulations, patients are seen for appointments with the pharmacist where the goals are to provide comprehensive medication management services that improve outcomes and provide affordable medication options. The ambulatory pharmacist contributes to the success of the program through the achievement of quality, clinical and reimbursement metrics. The pharmacist additionally contributes to improved prescribing within the clinic through formal education and academic detailing initiatives.
