General Statement of Duties: Provide coverage for open nursing shifts in assigned residential/ day habilitation programs. Assess, plan, evaluate and implement care according to accepted standards of practice.
Responsibilities:
Attend to the daily needs of individuals as well as foster increased independence and self-determination.Assist individuals with personal care tasks, which include but not limited to: toileting, showering/bathing, grooming, personal hygiene, assisting with mobility, repositioning, dressing, range of motion exercises, meal preparation and eating.Follow individual protocols, treatment plans, behavior plans, etc. When indicated, follow the Department of Public Health’s Medication Administration Program policies.Ensure safe and proper storage of medications; ensure medications are administered as prescribed; ensure documentation is completed as required. Review medical records and physician orders to ensure program compliance, updating information as needed.When indicated, work with community physicians and program staff to formulate and implement effective medical treatment plans.Assess, document and appropriately utilize necessary data, daily progress notes, staff log and complete all other and program required documentation.Advise program manager / nurse manager and staff of any medical problems that may hinder an individual’s progress or that may be aggravated by activities at the program.Consult with staff on methods of medical treatment and care.Transport consumers to and from day programs, medical appointments and other activities as required.Provide opportunities for leisure time activities and foster independence in the choice and participation of those activities.Maintain knowledge of ongoing program issues and developments as well as informing supervisor of program issues and concerns.Provide training and orientation for direct care / nursing staff as needed.Attend required trainings. Assist with admissions and discharges to and from hospitals and rehabilitation centers. Perform other related work duties as needed or as assigned by program manager, nurse manager or designee.Perform duties to reflect agency policies and procedures.Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.Ability to lift, transfer, push/pull, maneuver, and reposition 25 pounds and over.BAMSI conducts as needed, job-related background checks (e.g., may include but is not limited to fingerprints, drug testing, TB testing, verification of employment history and/or reference checks) prior to employment.
