Join our exceptional team and help us to create the strongest abdominal transplant program possible as we continue to provide the highest quality of health care possible to our communities. **Primary Function/General Purpose of Position** Facilitates/coordinates all pre and post abdominal transplant-specific phases of care and processes for transplant recipients (referral, evaluation, follow-up care). Maintains clinical documentation of requirements for transplant centers. Collaborates with all team members to provide individualized, multidisciplinary patient care. Participates in quality improvement efforts in alignment with program, department and hospital strategic plan. Monitor and coordinate care of the candidate awaiting transplantation. Exhibits ability to maintain and update candidate listing status per regulatory regulations. Identifies the need for candidate re-evaluation and initiates process when appropriate. Participates in patient and donor call coverage. Shares pertinent information about the intended recipient with the multidisciplinary transplant team. Facilitates admission or transfer of patient to the transplant facility. Notifies multidisciplinary transplant team of pending medical and/or surgical procedure. Add/Remove transplant recipient from waiting list according to regulatory guidelines.
