Full-time: 800 Main Street, Niagara Falls, NY SUMMARY OF POSITION FUNCTION: The ACT Team Nurse provides overview in all physical and behavioral health issues of individuals receiving services from the team. The nurse provides leadership and guidance to the team in addressing all preventative, ongoing and critical health matters. The nurse also coordinates pharmacological services along with the team’s prescriber(s). Nurses are core members of the team and contribute to all clinical decisions in the team, including treatment plans and documentation. MAJOR DUTIES AND RESPONSIBILITIES: + Knowledge and demonstration of agency core values in day-to-day activities + Conduct comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness; past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol, education and employment, social development and functioning, activities of daily living (e.g., self-care, living situation, nutrition, money management), and family structure and relationships + Manages all aspects of the psycho-pharmacological treatment of ACT clients, in collaboration with, and under the direction of, the ACT MD/NP. + Administer injections and educate participants on medication adherence. + Coordinate laboratory services and reports. + Order and store medical supplies. Coordinate with pharmacies to ensure timely delivery of medication to the ACT office or the client’s home. + Document all services and progress notes in electronic health reporting. + Document all services and progress notes in electronic health reporting. Provide case management for clients including coordinating and monitoring the activities of the individual treatment team, assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in conjunction with the treatment team, providing individual supportive therapy and symptom management, ensuring immediate changes are made in the treatment plans as clients’ needs change, educating and supporting clients’ families, and advocating for client’s rights and preferences. + Makes appropriate community referrals for services as required such as referring participants to community physicians and maintain contact with primary care providers. + Lead conversation concerning physical health, medications and nutrition during morning meetings. + Develop and lead health education classes on topics such as diabetes, heart disease, substance use, asthma, nutrition, wellness, medication self-management. + Provides health education to ACT staff, providers, families, and significant others. + Participate in staff development trainings. SKILLS/COMPETENCIES: + Demonstrate clear understanding of the characteristics and problems of adults with severe and persistent mental illness and be knowledgeable about the ACT model + Demonstrate an understanding of team decision making and collaborative communication + Demonstrate thorough knowledge of effective clinical practices and principles + Demonstrate knowledge of psychotropic medications and the management and administration of medication + Strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work and have access to helpful adequate, competent, and continuous supports and services. + Must possess skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness and respect for clients rights and personal preference in treatment are essential + Exceptional interpersonal skills, judgment and decision making skills + Must possess excellent oral and written communication skills in English + Proficient in the use of computer software such as MSWord, Excel, Outlook, etc. EDUCATION REQUIREMENTS: + Master’s, Bachelor’s, Associate Degree of Diploma in Nursing, must be licenses as a Registered Nurse in the state in which the program operates EXPERIENCE: + Minimum of three (3) years of work experience in an inpatient or outpatient psychiatric setting, which assures sufficient knowledge and experience applicable this position. + Experience with a multidisciplinary mobile team would be helpful + Must possess a valid Driver’s License with a satisfactory driving record and possess a personal vehicle for job requirement. Training from ACT Institute within 6 months of hire COMPENSATION: $59,500/yr - $75,900/yr
