As a Community Support Specialist, you will work closely with individuals to develop personalized plans that address their unique needs and goals. You will provide evidence-based interventions, connect clients with community resources, and collaborate with other professionals to ensure comprehensive mental health support. Key Responsibilities:

Provide community-based mental health services, including assessment, diagnosis, treatment planning, and intervention implementation.

Collaborate with clients, families, and stakeholders to develop and implement individualized treatment plans.

Utilize a strengths-based approach that incorporates evidence-based practices such as Motivational Interviewing, Positive Reinforcement Techniques, Trauma-Informed Strategies, and skills from CBT and DBT.

Implement behavioral interventions, including motivational interviewing, operant conditioning, interpersonal and group dynamics, and cognitive processing.

Coordinate care with service providers, including physicians, psychiatrists, schools, and community organizations.

Maintain accurate and timely records of client interactions, treatments, and progress.

Participate in staff development activities, training programs, and quality improvement initiatives.

Collaborate with team members, including case managers, therapists, and support staff, to ensure comprehensive care.

Develop and maintain relationships with key stakeholders, such as parents, teachers, and counselors.

Facilitate PSR group sessions as relevant to client treatment plan goals.

[For ACT/ACT TAY/IMPART roles]: Demonstrate knowledge of the ACT model, core processes, and service delivery methods.

Perform other duties as assigned by the supervisor.
 

Education and/or Experience Qualifications:

Bachelor's degree in Psychology, Social Work, or a related human services field; or

Bachelor's degree in an unrelated field with two (2) years of related work experience; or

A four-year combination of higher education and two (2) years of related work experience; or

Associate’s of Applied Science in Behavioral Health Support; or

Four (4) years of qualifying experience*.
 

Qualifying experience must include service delivery to individuals with mental health disorders, substance use disorders, or developmental disabilities. Experience should include:

Providing one-on-one or group rehabilitation/habilitation services focused on recovery and resiliency.

Teaching and modeling coping skills, psychiatric management, and use of natural resources.

Supporting individuals in employment, family, school, and community functioning.

Assisting individuals in achieving goals outlined in their individual treatment plans.
 

Additional Qualifications:

Strong knowledge of case management, rehabilitation principles, and treatment modalities.

Understanding of community resources and legal procedures related to mental health and substance use treatment.

Effective verbal and written communication skills, with the ability to produce clear reports.

Self-motivated with strong problem-solving and multitasking abilities.

Ability to establish meaningful relationships with clients and families.

Capacity to work independently while maintaining strong communication with team members.

Ability to follow direction and accept supervision.
