Responsible for assessing the functioning level of individuals in the Brookwood program and developing and implementing an appropriate planned activity program of services for all adults based on this assessment. Responsible for the ongoing evaluation of the effectiveness of the program and for making any necessary revisions to offered programming. Develops, documents, and implements a variety of planned activities to provide an appropriate therapeutic recreational program for all individuals at Brookwood consistent with current skills and interest levels as detailed in the ISP of each person. Plans and implements in conjunction with management and transportation, recreation, and other necessary support departments. Develops and implements minimally a monthly schedule of meaningful activities to meet the needs and interests of the individuals in the program. The frequency of schedules and activities to be planned daily will be determined by the Brookwood management team. Plans and coordinates larger scale events for Brookwood to include events that incorporate individuals from other day programs, outside vendors, volunteers, and special guests. Delegates assignments to Direct Support Professionals (DSPs) and/or instructional employees in the implementation of activity programming. Plans community integration opportunities that incorporate the interests and needs of the individuals supported. Maintains consistent records of individuals’ interests, progress, participation, and attendance. Prepares and submits to the supervisor/case manager any and all required reports, as needed. Is fluent in computer use and programming for professional correspondence and documentation including utilizing email and an electronic health record (EHR) to run reports, share, seek, and record data, ensuring all necessary documentation for individuals (health, behavior, attendance, etc.) in the EHR is completed, up to date, and accurate. Completes all necessary documentation as assigned in accordance with Department of Aging regulations. Responsible for maintaining familiarity with applicable licensing regulations and funding agency requirements. Completes vehicle training and is responsible to transport groups of individuals in appropriate vehicles, which would include larger vehicles such as a Goshen, and assisting with transportation, both to and from the program. Responsible for following and providing support in accordance with the ISP and Behavior Support Plan (BSP), if applicable. Prepares, records, and submits accurate data to a designated team member(s) for all required reports according to due dates. Assists with individual arrival to and dismissal from the program. Provides ongoing assistance and support in the areas of personal care and activities of daily living (including meal times, bathroom, etc.) as stipulated in the ISP. Maintains a clean, safe, and organized work environment, which includes being respectful of shared space and the assurance of assigned inventory, needed supplies, and maintenance of all materials and equipment. Ensuring that materials and supplies are kept clean, sanitized, and in good working order. Assists with inventory, as assigned. Dedicated to promoting and assisting with creative programming throughout Brookwood. Initiates ideas for holidays, special events and seeks out opportunities beneficial to the individuals. Remains in tune and knowledgeable of trends in the field. Participates in the implementation of programming in the most appropriate environment, including residential homes. Attends and participates in training sessions, staff meetings, and special event assignments, as required. Responsible for meeting and maintaining all regulatory and Woods mandatory training. Serves on planning committees and actively participates in planning for larger-scale events across Woods programs. Willing to attend specialty classes/conferences, possibly outside of regularly scheduled work hours. Acts as a support to all Woods’ adult day programs, including participation in cross-training, if appropriate. Displays professionalism and maintains effective communication and relationships with all stakeholders, staff, and management. Performs all other duties as assigned to meet the needs and ensure the effectiveness and efficiency of the program. Proof of a High School Diploma, GED or College Degree is required. Must have experience working with adults with developmental disabilities. Must have a demonstrated ability in activity planning and implementation. Degree or certification in recreational therapy or related field preferred. Must be 21 years of age. Must have a car and valid driver’s license; proof of valid automobile insurance submitted annually. Must be able to lift a minimum of 50 pounds.
