Job Title: Activities Leader
Job Description

Develops and executes personalized care plans for participants, ensuring creativity and individualization. Trains and supervises activity aides, delegating tasks as needed. Designs and conducts diverse monthly programs, utilizing both group and individual interventions to enhance participants’ cognitive and functional abilities. Provides daily therapeutic activities for participants with special needs. Collaborates with the Activity Therapist and Specialist to organize monthly community outings and special events. Works with the Coordinator to recruit and orient volunteers for specific recreational activities. Records participant engagement in activities and updates care plans accordingly. Participates in committees, special projects, and tasks as assigned by the Activity Specialist. Engages actively in team discussions, contributing feedback, ideas, and suggestions both daily and in monthly department meetings. Consults with the supervisor and activity therapist regarding participant care plans.

This is a permanent position.

Work Site

This is a fully on-site position in San Jose, California.

Work Environment

Mon-Friday 8-4:30. The working environment may be in an ambulatory clinic, shared with other clinical staff such as RNs, PCPs, LVNs, CNAs, and MAs. A designated workspace is provided for documentation and administrative tasks. The work area should be maintained in a neat, orderly, and clutter-free condition. Must be able to wear personal protective equipment (gloves, mask, etc.).
