Novant Health is seeking a Clinical Pharmacist 2nd shift, to join our acute care pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

Essential functions include:

Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standardsCompletes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered careMakes appropriate evidence-based, patient- centered medication recommendationsParticipates in the management of medical emergenciesProvides discharge medication review, reconciliation, and counseling as appropriateMaintains proficiency in and actively engages in central dispensing functions and operational programsOversees pharmacy operations and technician practice/activitiesMaintains proficiency in preceptor roles and actively emulates education/ mentorship skills

Qualifications Education: 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification, preferred.
Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team. Responsibilities
