Performs a variety of clerical support services for the efficient operation of the Care Coordination Department.  Performs and provides personal Secretarial and Support Services such as maintaining files, minutes, manual, and preparing correspondence.

 

Responsibilities

 

Provides secretarial/administrative coordination support to VP of Medical Affairs, Corporate Director of Care Coordination and Managers of the Care Management Department.Answers telephone and direct calls appropriately and/or resolve issues with patients when appropriate/maintains voicemail for Care CoordinationOrganizes and compiles Physician Advisor timesheets for Corporate Director of Care Coordination and Administrative  signaturesOrganizes and compiles trauma physicians time sheets for administrative signatures and submits to financeFacilitates and records discharge days in LOS notebookAssist partners by accessing medical records for patient data reportingAssist department with office software/computer systems as neededFacilitates the Long Stay Rounds Reports and distributes to staff weekly Addresses PTO request and send to the appropriate managerMaintains processes associated with Mail for the department and Corporate Director Care CoordinationFacilitates PTO requests and Laborworxx requests for employees reporting directly to Corporate Director of Care CoordinationFacilitates and distributes Care Management Manager on-call schedules monthlyArranges meetings for Care Coordination, facilitating room requests, etc.Distributes faxes, automatic printouts and voicemail messages to appropriate individuals within Care CoordinationFacilitates Laborworxx entries (PTO, Sick, EA, etc.)Files as requested for Corporate Director of Care CoordinationAssists with distribution of Care Manager/Social Worker Daily Staffing AssignmentsFaxing for the Commercial UR staff as required/requestedAssists with IM Notices as required and requested.Assists with maintenance of Indigent Fund & Finn/Quinley SpreadsheetMaintain Ambulance and taxi check requests and spreadsheets monthlyKnowledgeable of hospital mission, vision, and values and performs in a manner that supports them. Adheres to Policy and Procedures of the Care Management Department

Schedule meetings/meeting spaces/catering at the request of Managers/DirectorSchedule external partner meetingsAgenda’s & Minutes for meetingsOrder and Maintain supplies for Care Management RMC/NMC Maintain the Care Management Intranet pageCreate and update forms as neededPrepare and send records to ChartmaxxPrepare PowerPoint presentations and spreadsheets as needed

Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.Performs compliance requirements as outlined in the Employee HandbookAdheres to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.Requires use of electronic mail, time and attendance software, learning management software and intranet.Adheres to all DCH Health System policies and procedures.All other duties as assigned.
