Raleigh Court Health and Rehabilitation in Roanoke, VA is seeking an Assessment and Admissions Nurse, to work Full or Part-Time (3P-11P), responsible for evaluating potential residents for admission into the skilled nursing facility, ensuring that each patient’s care needs are assessed accurately. The role requires a highly skilled, compassionate nurse with strong assessment, communication, and organizational skills. Key Responsibilities:
Assessment of Potential Admissions:
Conduct thorough initial assessments for prospective residents, including reviewing medical records, conducting interviews with family members, and communicating with referring healthcare providers.
Evaluate the physical, emotional, and cognitive status of potential residents to determine appropriateness for skilled nursing care.
Identify the specific care needs, including but not limited to, medical, rehabilitative, and personal care.
Admission Process:
Coordinate and manage the admission process, including completing required paperwork, ensuring all clinical documentation is accurate and up to date.
Provide potential residents and their families with information about the facility’s services, programs, and policies.
Facilitate and assist with patient and family orientation to the facility.
Ensure compliance with all regulatory requirements related to admissions.
Skills & Abilities:
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills to work effectively with residents, families, medical staff, and external partners.
Knowledge of state and federal regulations related to skilled nursing facilities, Medicare, and Medicaid.
Proficient in using Electronic Health Records (EHR) systems.
Ability to work independently and as part of an interdisciplinary team.

What you need
· Active Licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in state of practice.
