Job Description
The Admissions Nurse will coordinate and facilitate the admissions process for new residents (Elders), ensuring a smooth transition into our community. This includes conducting thorough assessments of prospective Elders to determine their eligibility and care needs. Collaborate closely with healthcare professionals and engage Elders and their families to develop personalized care plans that meet their individual requirements. Upholding compliance with regulatory standards and meticulous documentation is integral to my role, along with providing compassionate support and guidance to Elders and families throughout the admissions journey. Will maintain accurate and confidential records of Elders using electronic health record systems and actively participate in interdisciplinary team meetings to review progress and refine care plans to ensure optimal resident care and satisfaction.

Requirements
License/Credentials
LPN with current, unencumbered license to practice as a LPN in this state.
Education Requirements: High school diploma (or GED).
Minimum Qualifications
Must be at least 21 years of age; cannot be a resident of the facility; shall have verification that he/she is not listed on the "Mississippi Nurses Aide Abuse Registry"; full time hours required; LPN with current, unencumbered license to practice as a LPN in this state with a minimum of two years experience working in a hospital, long term care facility, or other health care institution, with training or experience in geriatrics or a graduate of a four year degree program with related experience in a geriatric related field.
