Job Summary
As an Adult Behavioral Health Counselor, you will:•Provide follow-up care and support for high-risk behavioral health patients (ages 18-64) transitioning from inpatient psychiatric care or emergency visits.
•Conduct post-discharge outreach and collaborate with community behavioral health providers to ensure seamless continuity of care.
•Deliver targeted, short-term care management interventions to help patients stabilize and connect with essential resources.
•Support MassHealth Medicaid quality metrics by ensuring timely follow-up care after mental health inpatient or emergency department discharges.
Qualifications
What We’re Looking For
Current licensure: LMHC, LCSW, or LICSW (Massachusetts).1-3 years of experience in counseling, behavioral health coordination, or working with SMI (Serious Mental Illness) or SUD (Substance Use Disorder) populations (preferred).
