In this Monday-Friday 8am-4pm position, we are seeking an RN to oversee our Adult Medical Day Services program.

Responsibilities Ensures that psycho-social needs are assessed, evaluated and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.Ensures the following program components are available:Case management, including health educationInterdisciplinary care planningNursing servicesNutritionSocial servicesAssistance with activities of daily livingPlanned therapeutic or recreational activitiesPharmaceutical servicesProvides social services ensuring registrant psycho-social needs are assessed, evaluated, and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.Ensures arrangements are made for the use of and/or access to other community resources as needed.Ensures a comprehensive assessment is completed upon admission and every six months thereafter per regulation.Ensures a clinical record for each registrant is kept confidential, intact, and readily availableSupervise direct care staff ensuring registrants receive assistance in accordance with their plan of care.Create a Monthly Activity CalendarManage staff to address ADLs and programming needs and documentation throughout the day.Manages and completes documentation of daily programs, nursing clinical needs, appointments, and ADLs daily.Monthly audits performed on documentation and assessments. These audits are submitted to the administrator.Weekly and Monthly reports are submitted to the administrator based on the following information:Nursing Clinical NeedsSocial leisure Program participationADLsMedical AppointmentsScheduling of upcoming appointmentsAssessment and documentation completion Qualifications Graduate of an accredited nursing school. Current active nursing license required (RN). At least one year of supervisory experience required.This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
