Join CentraCare as an Advance Care Planning Facilitator as part of our Senior Transitions Team. 

The Advance Care Planning (ACP) Facilitator provides patients with onsite advance care planning services, support, and assistance. This role serves as a dedicated point of contact for patients seeking assistance with advance directives across various locations to include Marketplace in Waite Park or any of our community clinics.  In addition, will provide patient education and engagement strategies to improve patient understanding of care planning, increase advance directive completion rates, and will respond to patient questions and concerns providing awareness, education, and individual assistance.

Schedule:

16 hours every 2 weeks Weekday hours 8am to 4:30pm, however hours may vary due to presentation needs and community events The duration of this opportunity is temporary with an anticipated end date of December 31, 2025. 

Pay:

Starting pay $17.83 per hour Pay range $16.59 to $24.92 per hour depending on years of relatable experience. 

Requirements: 

High School Diploma or GED  Demonstrated skills including customer service, problem solving, communication, organization

Preferred Qualifications:

Bachelor's Degree Experience working in health care settings.
