Position Summary: Delta Family Medicine is looking for a Physician Assistant or Nurse Practitioner to join our growing practice. Located in Delta, Utah this rural setting will offer a variety of care paradigms from newborn to geriatric.
Essential Job Functions:
1. **Patient Care and Management**: - Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic evaluations. - Develop, implement, and manage treatment plans for acute and chronic illnesses. - Prescribe medications and therapies, including lifestyle modifications, as appropriate.
2. **Diagnostic Testing and Interpretation**: - Order and interpret diagnostic tests such as lab work, imaging studies, and other specialized tests. - Perform routine in-office procedures like suturing, wound care, and minor surgical interventions.
3. **Preventative Care and Health Education**: - Provide patient education on health promotion, disease prevention, and management of chronic conditions. - Administer vaccinations and other preventive measures.
4. **Collaboration and Coordination**: - Collaborate with physicians, specialists, and other healthcare providers to ensure comprehensive patient care. - Coordinate patient referrals to specialists, hospitals, or other healthcare facilities when necessary.
5. **Electronic Health Records (EHR) Management**: - Document patient encounters, treatments, and progress notes accurately and promptly in the electronic health record system. - Ensure compliance with relevant laws and regulations regarding patient confidentiality and record-keeping.
6. **Community Health Engagement**: - Participate in community outreach and health promotion activities to improve public health in the rural setting. - Engage in local health initiatives and programs to address the unique healthcare needs of the rural population.
7. **Emergency Care**: - Provide initial emergency care and stabilization in acute situations until transfer to appropriate emergency services is arranged. - Be prepared to handle urgent and emergent cases independently due to the rural setting's limited access to immediate specialist care.
8. **Continuing Education and Professional Development**: - Stay updated with the latest medical advancements, evidence-based practices, and continuing education requirements. - Participate in ongoing professional development and training programs.
9. **Quality Improvement**: - Contribute to quality improvement initiatives within the clinic to enhance patient care and clinic operations. - Participate in regular clinic meetings and provide input on practice policies and procedures.
10. **Patient and Family Engagement**: - Build strong, trusting relationships with patients and their families, understanding their unique needs and concerns. - Advocate for patients and assist them in navigating the healthcare system.
Qualifications:
1. **Educational Background**: - Master’s degree or higher from an accredited Nurse Practitioner program (for NPs). - Master’s degree or higher from an accredited Physician Assistant program (for PAs).
2. **Board Certification**: - Current board certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP) (for NPs). - Current board certification as a Physician Assistant by the National Commission on Certification of Physician Assistants (NCCPA) (for PAs).
3. **State Licensure**: - Current and unrestricted license to practice as a Nurse Practitioner or Physician Assistant in the state where the clinic is located. - If applicable, eligibility for licensure in the state if currently licensed in another state.
4. **DEA Registration**: - Current Drug Enforcement Administration (DEA) registration number for prescribing controlled substances. 5. **Basic Life Support (BLS) Certification**: - Certification in Basic Life Support (BLS).
6. **Experience**: - Minimum of 2-3 years of experience in a primary care or family practice setting, preferably in a rural or underserved area.
7. **Skills and Competencies**: - Proficiency in conducting comprehensive physical exams, diagnosing and treating acute and chronic conditions, and performing minor procedures. - Strong interpersonal and communication skills to effectively interact with patients, families, and healthcare team members. - Ability to work independently and manage a diverse patient population with varying healthcare needs.
8. **Professional Attributes**: - Demonstrated commitment to patient-centered care and health promotion. - Willingness to participate in community outreach and health education activities. - Flexibility and adaptability to handle the unique challenges of a rural healthcare setting.
9. **Technology Proficiency**: - Familiarity with electronic health records (EHR) systems and proficiency in documentation.
Hours: M-Fr, 9:00am - 5:00pm
