Located on the campus of Scripps Mercy Hospital San Diego, Scripps Ambulatory Clinical Pharmacy Services is a system-wide team of Advanced Practice Pharmacists. They provide top-of-license clinical management to our specialty patient populations, including Osteoporosis, Primary Care, HIV, Cardiology, Nephrology and Neurology. Understanding that the treatment of diseases extends beyond just medications, we focus on lifestyle and prevention counseling, in addition to the pharmacologic aspects of care. The aim is to get to know what is important to our patients, define a set of goals, and work together to achieve those goals and enhance a person’s overall quality of life. Scripps Ambulatory Clinical Pharmacy Services collaborate with physician specialists to assist with improving the long-term health and quality of life of our patients. The Advance Practice Pharmacist is key to providing care as it relates to patient medication needs.
The Advance Practice Pharmacist:
Assumes clinical pharmacy leadership responsibilities for area of specialized care.Active in system clinical program planning.Provides direct patient care as a provider under the privileges and credentialing program of Scripps Health pharmacy department.Operates under a collaborative practice agreement in the area of specialization identified.Participates in evaluation and management of diseases and health conditions including: performs patient assessments; orders and interprets laboratory values; refers patients to other providers when indicated; initiates, adjusts, modifies or discontinues drug therapy within the scope of the collaborative practice agreement and area of specialization.Collaborates with healthcare team to provide optimal medication management for patients within defined practice area.Evaluates the clinical practice for patient care and monitors processes for continuous improvement.Participates in research, publication and presentations at state and national organizations.Participates in the Peer Review process and oversees quality assurance in defined area of practice.Precepts residents and students when availableThis is a pharmacy leadership position functioning to advance practice within a designated scope of responsibility.This is a full-time position with day shifts, Monday through Friday, at our ambulatory clinics, primarily located at Scripps Mercy San Diego in Hillcrest. California Pharmacist LicenseAHA BLS for the health care providerBoard of Pharmacy Specialties (BPS) or other nationally accredited certification required within 2 years' of hire.Must be eligible to apply for the Advance Practice Pharmacist license within 1 year of hire and obtain license within 2 years of hire.Preferred Education/Experience/Specialized Skills/Certification:
Experience in a multi-site health system pharmacy enterprise;Experience using Epic in an ambulatory areaTeaching certificate and/or experience precepting students and/or residentsRequired Education/Experience/Specialized Skills:
BS Pharmacy or Pharm.D.
