Advanced Care Planning Coordinator - Population Health - System Wide

Our hospitals, care sites and clinicians are recognized for clinical and operational excellence. **Summary of Primary Function/General Purpose of Position** This position will provide direct administrative assistance to the Advance Care Planning (ACP) program. Will interact with patients at times to assist in scheduling ACP conversations, ACP document completion and/or entering patient ACP information and documents in EHR. **Essential Job Functions** Provides community based support and assistance to patients and staff to implement advance care plans as deemed necessary by the Advance Care Planning Team Assist clinical staff in for preparing patient appointments (pre-visit planning) Act as a resource to patients and families, providing answers to frequently asked questions and suggesting educational materials using ACP Program Protocols, within scope of role, training, and at the direction of a licensed clinician. Maintain comprehensive progress notes for each patient interaction Maintains good working relationships with and knowledge of applicable ACP services Utilize interpersonal skills and community ACP-related resources to effectively advocate for individual in the communities served Maintains required data and documentation and prepares required report for the Advance Care Planning Team Perform duties with minimal supervision. Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures. Adults (18-64 years). Geriatrics (65 years and older) **Employment Qualifications** Required Minimum Education: HS/GED Specialty/Major Preferred Education Associates Degree Specialty/Major Healthcare Related Field **Licensing/ Certification** Licensure/Certification Preferred LPN or Medical Assistant **Minimum Qualifications** Minimum Years and Type of Experience 1 year experience in community care, ambulatory care, or inpatient care setting. Other Knowledge, Skills and Abilities Required Works well in a Team Setting. Excellent interpersonal communication skills. Strong analytical, data management and computer skills. Experience with data entry and tracking. Highly organized and detail oriented.
