We work directly with our providers and nurses in a decentralized model and are extremely active in monitoring, adjusting, and recommending pharmacologic therapies. As a Pharmacist at Grand Strand, you will feel valued and practice at the top of your license._ **Qualifications** + **Licensure/Certification/Registration:** **Must have a valid active South Carolina registered pharmacist license.** + **Must meet one of the following criteria:** + **ASHP Accredited PGY-2 Pharmacy Residency Program** + **ASHP Accredited PGY-1 Pharmacy Residency Program + Pharmacy Board Certification** + **At least 5 years of hospital practice in the direct patient care setting + Pharmacy Board Certification** + **Education:** **Doctor of Pharmacy degree from an ACPE-accredited School of Pharmacy and a current pharmacist license in good standing with the appropriate State Board of Pharmacy** **Job Summary:** The Advanced Clinical Pharmacist is responsible for providing safe, effective, evidence-based, and cost-effective medication therapy management for acute care patients. They apply advanced knowledge, skills, and appropriate clinical judgment in all aspects of their work. They serve as a patient advocate when providing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. In addition, they promote the advancement of clinical pharmacy services within the hospital by serving on committees, participating in protocol and order set development, and providing education to colleagues and medical staff. They are primarily responsible for the direction of an assigned service line within the hospital and maintain an ongoing, self-motivated commitment to the advancement of pharmacy practice. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made by the clinical pharmacist. **Job duties:** + **Patient Care Activities including but not limited to:** + Multidisciplinary/bedside rounding + Medication and drug therapy/monitoring + Clinical services + Provides communication and consultation with practitioners and medical staff inside and outside of the pharmacy + Issuing controlled substances to patients + **Medication Management including but not limited to:** + Completion of medication use evaluations and drug utilization reviews + Participation in facility committees + Implementation of clinical services + formulary management and the continuous quality review of orders set + protocols and guidelines to ensure the practice of current/evidence-based practices + **Mentor/Educator** + Participates in training and mentorship of pharmacists, pharmacy students, pharmacy residents and pharmacy techs. + Participates in pharmacy preceptor development initiatives at least quarterly + Assists with conflict resolution with pharmacy and other disciplines + Assists with conflict resolution with pharmacy and other disciplines + Serves as the pharmacy liaison for the hospitalists and interdisciplinary team educator for pharmacy-led initiatives and changes **Benefits** **Grand Strand Medical Center, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
