Advanced Practice Nurse

Position Summary

The Advanced Practice Nurse is responsible for the day to day coordination and oversight of the clinical management of assigned patients in the outpatient Geriatric Assessment Program, nursing homes and sub-acute facilities. The Advanced Practice Nurse functions independently, while also collaborating with the interdisciplinary clinical team. They will be responsible for contributing to the ongoing development of clinical practice and standards of care within the service, as well as policies, protocols, guidelines and service developments. They will ensure the safety and well-being of patients and work in a manner that promotes dignity and human rights through patient centered care principles. The delivery of Geriatric Care will be enhanced through such activities as education, program development, quality improvement activities, consultation and care coordination.

Primary Position Responsibilities

1.Provide specialized comprehensive geriatric assessments and follow-up in the outpatient setting, as part of the interdisciplinary geriatric team.

2. Provide inpatient geriatric consultations

3. Provide primary geriatric care services in a collaborative environment for frail and elderly patients in sub-acute, assisted living and long-term care settings.

4. Review and order medications, labs and other diagnostic testing

5. Participate in on-call coverage in coordination with the department

6. Provide primary care that will focus on assessment, health management, education, advocacy and prevention.

7. Assist patients and families in identifying goals of care and in advanced care planning

8. Improve quality and efficiency of health care delivery

Qualifications

Minimum Education:

Required:

⚬Master’s Degree in Nurse Practitioner Program, National Certification as a Family or Adult Nurse Practitioner.

Preferred:

⚬ Additional courses in the care and treatment of Adult and Gerontology


Minimum Years of Experience (Amount, Type and Variation):

Required:

⚬ 2 years as a Nurse Practitioner

Preferred:

⚬ Hospice and/or Palliative Care Experience


License, Registry or Certification:

Required:

⚬ Current and valid New Jersey Registered Nurse license and a current and valid New Jersey Advanced Practice Nurse License, along with national certification as a Family or Adult Nurse Practitioner. Must also have a New Jersey Controlled Dangerous Substance (CDS) License and Drug Enforcement Agency (DEA) License

 


Knowledge, Skills and/or Abilities:

Required:

⚬ Strong Medical Knowledge Base in requisite specialty, Interpersonal skills and management ability provide patient care.

Preferred:

⚬ Previous clinical experience in working in long term care
