Advanced Practice Pharmacist, Ambulatory Multi-Specialty Clinic
We are so glad you are interested in joining Sutter Health! **Organization:** CPMC-California Pacific Med Center Davies **Position Overview:** Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, Continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies.
