**Location:** **METROHEALTH MEDICAL CENTER** **Biweekly Hours:** **80.00** **Shift:** **8a-5p** **The Division of Nephrology for the MetroHealth System is seeking a dedicated Nephrology APP to join our collaborative team in managing patients with acute and chronic kidney conditions.** **Job Description** **In this role, you’ll be an essential partner in providing comprehensive kidney care to adult patients receiving hemodialysis. This includes:** **· Routine rounding at local outpatient dialysis centers** **· Managing the day-to-day clinical needs of hemodialysis patients** **· Providing education to patients and family members about kidney disease, treatment options, and lifestyle modifications related to kidney health** **· Working closely with nephrologists, dialysis staff, and the interdisciplinary team to provide optimal care for our patients** **This is an excellent opportunity for an APP who thrives in a collaborative environment, values long-term patient relationships, and is eager to make a difference in the lives of individuals with complex kidney conditions.
