NSCH has ten services sites: Gloucester, Peabody and Salem Family Health Centers; the school-based health centers at Salem High School and Peabody Veterans Memorial High School; and behavioral health only services at the Bentley Academy, Collins Middle School, Horace Mann School, Bates Elementary School, and the Peabody Learning Academy. Primary Purpose of Position Under the general direction of the Chief Medical Officer (CMO) and the direct supervision of the Medical Director, the Per Diem Advanced Practice Provider (Nurse Practitioner or Physician Assistant), in collaboration with the physician(s) on the team, will provide continuing and comprehensive health care for the individual and family across all ages and genders. The Per Diem Advanced Practice Provider is expected to function as integral part of the Primary Care Team (PCT), sometimes as a leader and sometimes as engaged member. Essential Functions: · Provide direct care to scheduled and walk-in patients, including conducting exams, diagnosing, and administering treatments in accordance with accepted standards of care. · Perform history-taking, physical exams, and utilize diagnostic services (e.g., lab tests) in an ambulatory setting to inform treatment plans. · Conduct patient visits both in person and via telemedicine at multiple locations (Salem, Peabody, Gloucester), and refer patients to specialists when necessary. · Maintain patient confidentiality in compliance with NSCH Confidentiality and HIPAA guidelines while ensuring a professional and welcoming environment. · Collaborate as a key member of the PCT, supporting team efforts, enhancing customer service, and contributing to quality initiatives. Requirements Requirements: · Nurse Practitioner (NP) - Must be nationally certified as an FNP or AGNP, graduate from an accredited master's degree NP program and possess state licensure to practice as an APRN and maintain current license to practice professional nursing in the Commonwealth of Massachusetts · Physician Assistant (PA) - Must have national certification and Commonwealth of Massachusetts licensure as a Physician Assistant · NP/PA – Must possess and maintain, or be eligible for, Mass DPH Controlled Substance registration and DEA registration · Two plus years of experience in community or ambulatory care. Preference will be given to bilingual/bicultural individuals who are Spanish or Portuguese. · Strong communication and interpersonal skills, with commitment to establishing a safe and respectful environment for team members · Ability to work in a culturally diverse environment as part of a team · High level of commitment for comprehensive and quality health care · Excellent communication and organization skills with attention to detail
