Job Description​

We are hiring an experienced APP for our Mission Farms Primary Care Clinic in Overland Park, KS.

This position is accountable for advanced nursing practice in the care of assigned patient populationsResponsible for collecting and analyzing client health dataIdentifies appropriate expected outcomesDevelops and implements patient plan of careProvides comprehensive clinical coordination of care and case managementProvides consultation to influence plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulationsEvaluates clients progress in attaining expected outcomesConducts nursing research and provides education to patient family and nursing staff

Candidates with 2-3 years of advanced practice preferred.

Applicable Experience:

2 years

Advanced Practice Provider Cert(MO/KS) - Various, Basic Life Support - American Heart Association or Red Cross

Master's Degree
