Overview Responsibilities: As an outpatient palliative care advanced practice provider, you will provide compassionate and holistic care to patients with serious or life-limiting illnesses and their families. You will work collaboratively with a multidisciplinary team of physicians, nurses, social workers, chaplains, and other specialists to address the physical, emotional, spiritual, and social needs of patients and caregivers. A candidate for this position will: Perform comprehensive assessments Develop and implement individualized care plans, prescribe medications Order and interpret diagnostic tests, and provide education and counseling You will also participate in quality improvement initiatives, research, and education activities related to palliative care.
