Community Health Network has an excellent opportunity for an experienced, family medicine certified advanced practice provider to practice with our Virtual Primary Care team in a Full Time role. 

·       Remote work only; no call

·       Autonomy in providing patient care

·       Electronic Medical Record (Epic)

License & Certifications

·       Minimum two years of experience as an advanced practice provider is required.

·       Educational background consistent with requirements for licensure and certification with the State of Indiana
