As an After-Hours Registered Nurse, you’ll provide high-quality care and support to patients and their families outside of typical business hours. Your responsibilities will include:
Patient Assessments: Evaluate physical, emotional, and environmental needs of home care patients.Care Planning: Develop, implement, and adapt care plans for optimal patient outcomes, ensuring each visit reflects patient-specific needs.Communication: Relay important patient health changes, safety concerns, and other observations to the clinical supervisor, attending physician, and healthcare team.Patient and Family Support: Counsel patients and families on care techniques, safety, and available community resources.Plan of Care Management: Verify and follow individualized care plans, based on physician orders, patient assessments, and care guidelines.Quality Assurance: Ensure continuity of care, monitor patient progress, and address any unexpected changes in condition.Requirements:
Education: Graduate from an accredited nursing program (Certificate, Diploma, Associate’s, or Bachelor’s degree).Experience: Minimum of one year as a nurse in a clinical setting; home health experience is a plus but not mandatory.Licensure: Current Registered Nurse license in the state of practice.Skills: Strong communication skills and the ability to work independently.Mobility: Must be able to travel up to 50% of the time.Are you ready to bring high-quality care and compassion to patients in the comfort of their homes, especially when they need it most?
