Cleveland Clinic’s Social Work team provides caring, patient and family-focused social work services to meet basic human needs, enable clients to reach their optimal level of psychosocial functioning and enhance well-being. This position is critical to the delivery of quality psychosocial care and management of patients receiving cancer care at our regional sites.
A caregiver in this position works the day shift from 8:00AM to 5:00PM.
A caregiver who excels in this role will:
Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
Perform psychosocial assessments and provide interventions to assist patients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings.
Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
Maintain a network of resources for consultation and connect patients with community resources.
Supervise social work interns and assist with training new caregivers.
Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
Minimum qualifications for the ideal future caregiver include:
Master’s degree in social work from a CSWE accredited program.
State Licensed Social Worker (LSW).
Two years of experience in a hospital or healthcare setting.
Licensed Independent Social Worker (LISW) within five years of hire.

Physical Requirements:
Manual dexterity to operate office equipment.
May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
Must have normal or correction vision
Must be able to clearly communicate verbally by phone.
