the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational safe evidence based and cost effective therapy In addition to direct patient care responsibilities the clinical pharmacy specialist will actively participate in committees and or workgroups collaborate in the review of new and existing medications clinical practice guidelines and other related policies used in the management of the clinic specific medical conditions This person also conducts clinical research and practice advancement projects as well as patient care pharmacyExperiencebull PGY pharmacy training or three years equivalent experiencebull PGY residency preferredLicensure Certification Registrationbull Current unrestricted pharmacist license from Connecticut Board of Pharmacy or eligibility within daysbull Board Certification within specialty practice within months of eligibilitybull Ability to work collaboratively as part of a healthcare
