Annual Wellness Visit Coordinator

Growing together.** **Position Details:** **Location:** Colonial Healthcare, a part of Optum - 674 W Liberty Street, Sumter, SC **Schedule:** Monday - Friday, 8am-5pm / No evenings, weekends, or holidays! **Specialty:** Family Medicine **Primary Responsibilities:** + Look through the schedule for that day and make sure no patients were missed and need to be added. If so, you will need to add that patient onto your schedule + Research patient's most recent immunizations, procedures, scans and lab work listed on the schedule + Verify patients' insurance through Palmetto GBA and figure out which template should be used whether it will be: IPPE G0402 (First 12 months of having Medicare), Initial G0438 (12-24 months of having Medicare) or Subsequent G0439 (24+ months of having Medicare) + Check to see if the patient has had any more immunizations at another location on Simon and document that into your research + Check to see if the patient has a Mammogram, Dexa scan, Colonoscopy, Lab work and Immunizations listed in Epic that has not been sent over already + Request Medical records from other Healthcare offices if needed + Communicate with the patient about research already discovered before their appointment and that has not been found + Communicate with patients at the time of their appointment about Opioid use, Alcohol use, Tobacco use, depression, pain, dizziness, vertigo, being independent when it comes to their daily activities and their functional ability + Voice to the patient and provider about what the patient is currently due for + Verify patients' medical history, social history, hospitalization history, surgical history and family history + Double check about certain medications the patient is currently taking + Measures BP, Height, Weight and BMI + Document in patient Annual Wellness Visit note all the information you researched and all the new information the patient advised you about when performing the AWV + Route the note over to the patient's established provider so they can visualize your work and add any more details that may have been missed. Once that is complete the provider will lock your note so it can be sent over to the billing department + Fill out the ACO Quality Measures Form with what information applies + Scan the ACO Quality Measures Form into patient’s chart in preferred folder You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. **Required Qualifications:** + High school diploma or completion of an equivalent adult education program + Active medical assistant certification (CMA, CCMA, NCMA or RMA) + 2+ years of experience in a family practice setting + Experience with Medicare patients The hourly range for this role is $16.00 to $28.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
