APP - AHMG Primary Care at Porter

AMHG Primary Care at Porter is seeking an experienced NP or PA to join our growing system of care.

Open to New Grads or Experienced

Documents ongoing status, interventions, patient response and outcomes in accurate, timely manner.

Engages patient/family to foster commitment to comply with plan of care.

Collaborates with interdisciplinary team to develop and implement holistic, individualized plan of care.

Evaluates patient’s response and intervene in a timely manner to ensure optimal patient outcome.

Implements patient plan of care and observes outcomes in accordance with nursing standards and physician direction.

Serves as patient advocate. Anticipates needs of provider, staff patient and family.

Effectively uses time, personnel, equipment and supplies to provide high-quality, cost-effective delivery of patient care.

Education and Experience Required:

Master’s Degree in Nursing as Nurse Practitioner, from a nationally accredited postgraduate certificate program. Must be a graduate from an Accredited School of Nursing.

Master's degree in physician assistant studies as a Physician Assistant, NCCPA certification

Licensure & Certificates Required:

Registered Nurse Practitioner Licensure for Colorado; and listed on the state Advanced Practice Nurse (APN) Registry.

Board certified as an Advanced Practice Registered Nurse

Registered Physician Assistant Licensure for Colorado

has received your NCCPA certification for Physician Assistant

BLS - American Heart Association

ACLS - American Heart Association

Salary Range: $144,745-$158,834
