SUMMARY

The primary purpose of the Diagnostic Imaging (DI) Advanced Practice Provider (APP) position is to provide diagnostic and therapeutic patient care under the direct supervision of a physician. The scope is patient management before, during and after diagnostic and therapeutic radiological procedures/treatments in both inpatient and outpatient settings. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.

JOB SPECIFIC COMPETENCIES

Patient Clinical Assessment, Management, and Documentation
The APP will be expected to respond to any event in Diagnostic Imaging (DI) that constitutes an emergency. This can include but not limited to falls, difficulty breathing, allergic reactions, contrast reactions, port extravasation, incidental cord compression, seizure, glycemic event, etc. Provider will assess the patient through patient interview and pertinent physical exam, treat if possible, and disposition the patient. Patient satisfaction will be derived from above average interpersonal skills needed to provide thorough patient and family education. The APP will be expected to manage post-procedure/post-imaging care for outpatients and inpatients as needed. Outpatient post-procedure/post-imaging care may include coordinating with primary service for subsequent procedures and repeat imaging as well as providing appropriate expectations and instructions for follow-up. The APP will be expected to perform history and physical examinations focused on the specific body region/system(s) relevant to the diagnostic imaging procedure/treatment. In this role, knowledge of human anatomy and physiology must be appropriate and complete. The APP will be expected to perform consults and follow up visits, complete H&P and pre-treatment assessments specific to different DI treatments, therapies, and procedures. The APP will see patients in the clinical outpatient setting prior to diagnostic imaging procedures, treatments, and therapies, including, but not limited to, patients for Nuclear Medicine and Breast Imaging departments within Diagnostic Imaging (DI) Division. The APP will may perform sedation assessment, relevant to institutional/departmental policies, including ASA scoring, airway classification, ECOG scoring, medication management and medical referrals. The APP will order and interpret pertinent laboratory and diagnostic studies/imaging. The APP will prescribe medications, determine pre, intra, and post procedure medical management, and write orders within their licensed scope of practice. The APP will formulate/execute plans for management of common DI periprocedural issues, including but not limited to: lab work, transfusion, contrast, anticoagulation/antiplatelet, and post procedure pain management issues. The APP will assess pre-procedure medical management issues such as periprocedural hypertension/hypotension, fluids/electrolytes imbalances (critical values), and unmanaged diabetes (insulin pump, diabetes management, hyper/hypoglycemia). The APP will be expected to formulate a management plan with the DI physician and primary team regarding all aspect are periprocedural/therapy administration management. The APP will demonstrate the ability to triage cases appropriately. The APP will practice within E&M Billing and Compliance regulations and requirements and demonstrate ability to complete appropriate documentation. Provides clear written and verbal communication. Provider will be responsible for protocoling imaging studies ordered by non-radiology providers throughout the institution to ensure proper imaging is obtained. Participates and collaborates in clinical research protocols. Research duties including clinical/data collection, management and obtaining informed consent when delegated to do so. Other duties as assigned.
Patient Education, Post-procedure Follow-up, and Longitudinal Care

Provides detailed procedure education, including specific procedure risks, benefits, alternatives and expected outcomes. Participates in the informed consent process per institutional policy.
Provide patient education to both patients and their families regarding procedure preparation, expectations, recovery and post-procedure care and follow-up. Outpatient post procedure longitudinal efforts include coordinating with primary service for subsequent procedures/treatments, arranging repeat imaging/labs and establishing follow-up schedule/expectations. Phone, video, and/or in person evaluations are performed to assess for complications, rate/record any adverse events, and ensure appropriate follow-up course is completed. Patient satisfaction will be derived from above average interpersonal and communication skills. The PA will also participate in the development of patient education materials.

Practice-Based Learning and knowledge

Holds an integral role in the patient management team. This will include above average telephone and personal communication skills for consultation with patients, physicians, nurses and other medical team members.
The Diagnostic Imaging Advanced Practice Provider also is called to evaluate patients in recovery to identify and manage post procedure problems. The APP is expected to formulate a plan with the DI physician and primary team regarding subsequent management and or procedure(s). Will also be involved in catheter and procedure related issues and/or complications. Understanding the role of imaging techniques and following guidelines and policy to obtain the best diagnostic imaging studies and images.

REQUIREMENTS

EDUCATION

PA - Required: Graduate of a Physician Assistant training program approved by the Council of Medical Education of the American Medical Association.
APRN - Required: Master's Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants who obtained recognition as an Advanced Practice Registered Nurse from the Texas State Board of Nursing prior to 1/1/2003. Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population

EXPERIENCE:

Required: None
Preferred: Advanced Practice Provider (APRN or PA) with 1 year of experience.

LICENSURE/CERTIFICATION:

PA - Required: All of the following:
PA certification by the National Commission of Certification of Physician Assistants. Licensed by the Texas State Board of Physician Assistant Examiners.

Must meet the American Heart Association Basic Life Support (BLS), ACLS (Advanced Cardiac Life Support) or PALS (Pediatric Advanced Life Support) if working in one of following department: Pediatrics - Patient Care (113100).

APRN - Required: All of the following: Written authorization from the Texas Board of Nursing to function as an Advanced Practice Registered Nurse. Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification. Pediatric Advanced Life Support (PALS) If working in the following departments: Pediatric - Patient Care (113100).

Eligible to be granted limited prescriptive authority by the State of Texas. Authorization for limited prescriptive authority by the Texas Board of Nursing.
