Associate Director Telephonic Clinical Operations Care Coordination
**Become a part of our caring community and help us put health first** The Associate Director, Home Health Telephonic Care, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Home Health Telephonic Care Nursing Associate Director requires a solid understanding of how organization capabilities interrelate across department(s). The **Associate Director, Home Health Telephonic Care** : + Employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. + Identifies and resolves barriers that hinder effective care. + Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. + May create member care plans. + Decisions are typically related to identifying and resolving complex technical and operational problems within department(s). + Leads multiple managers and/or highly specialized professional associates. **Use your skills to make an impact** **Required Qualifications** + Bachelor's Degree in Nursing + 6 or more years of nursing experience + 2 or more years of management / supervisor level experience + Licensed Registered Nurse (RN) in the (appropriate state) with no disciplinary action + Progressive business consulting and/or operational leadership experience + Comprehensive knowledge of all Microsoft Office applications including Word, Excel and PowerPoint + Must be passionate about contributing to an organization focused on continuously improving consumer experiences **Preferred Qualifications** + Master's degree in a health-related field + Case Management certification (CCM) + Previous experience working in a managed care field **Additional Information** **To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided** **internet** **service of Home or Hybrid Home/Office employees must meet the following criteria:** + At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. + Satellite, cellular and microwave connection can be used only if approved by leadership. + Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for theirinternetexpense. + Home or Hybrid Home/Office employees will be provided with telephone equipment appropriate to meet the business requirements for their position/job. + Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. **Scheduled Weekly Hours** 40 **Pay Range** The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $104,000 - $143,000 per year This job is eligible for a bonus incentive plan.
