Asst. Manager, Care Management

Acts as a support to the Manager in the daily management of the department. 

Manages the daily workflow and work schedule for Case Management.  Assists the Manager in assuring continuity and quality of service and care in collaboration with Overlook's care coordination process. In the absence of the Manager will fulfill those responsibilities essential to the department's effective operation. Collaborates with the SW Supervisor to coordinate time off to ensure consistent coverage for the department. Assists office staff with problem solving

Acts as a resource to the case management staff.

In collaboration with the Manager, makes recommendations for growth, learning needs and assures that performance meets standards of practice.  Provides a skilled, resource to the staff, responsible for daily assignments Intervenes in situations of staff availability, crises  Attends interdisciplinary rounds on an ongoing basis Mentors and educates case managers Provides guidance and supervision to case managers.  Collaborates with manager for performance improvement plans or disciplinary plans.
