Job Description:

The Health Coach works in conjunction with Healthy Connections department Care Management team to provide individualized assistance to members related adoption of healthy behaviors. The Health Coach role works with members to make behavioral changes that will improve health status, reduce health risks and improve quality of life to overcome identified barriers. The Health Coach collaborates and creates an exceptional experience through facilitation of timely access to health promotion and wellness services, delivery of education resources, administering additional health and wellness needs assessments, and providing frequent coaching sessions. The Health Coach works with members to ensure personal goals of care are addressed allowing them to experience their most optimal health through all phases of the Care Process Model experience.

Scope

As an Athletic Trainer you need to know how to

Identify the needs and develop programs and services within the budget constraintsEducate the athlete and/or patient and family as part of the treatment plan and rehabilitative processDocumentation of all healthcare for injury tracking systemPerform injury evaluation for any injured athlete/patient.Make independent decisions to develop and modify specific treatment plans for each individual injury and rehabilitation for each injury.

Minimum Qualifications

Bachelor's degree, requiredCurrent licensure by the Montana State Board of Athletic Trainers (LAT),requiredCurrent BOC certified status, requiredCurrent BLSCertification endorsed by the American Heart Association, required upon hire.

Preferred

Masters degree, preferred

Physical Requirements:

Excellent communication skillsOperate computers, telephones, office equipment, including manipulating paper requiring the ability to move fingers and hands.See and read computer monitors and documents. Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$19.97 - $29.74
