General Summary
Develops, implements and manages the Heart Rhythm Disorders program. Acts as a liaison between all providers involved, the service line, medical group and other related programs in the system. The candidate in this role will split time between WellSpan's Cardiology practice/s in Lebanon and Lancaster county.
Duties and Responsibilities
Essential Functions:
+ In collaboration with the Electrophysiologists and the Cardiovascular Service Line, assumes responsibility for the development, implementation and management for clinical care relating to heart rhythm disorders
+ Works closely with electrophysiologists to assess the needs of the patients, establish a service that will provide ongoing support and advice for patients and family members.
+ Participates in the development, maintenance, implementation and evaluation of written care protocols based on established guidelines from the American Heart Association Get with the Guidelines and the American College of Cardiology, including physician orders, clinical pathways for expected length of stay and clinical practice guidelines.
+ Ensures data is collected, reviewed and analyzed following the American Heart Association Get with the Guidelines protocol. Develops and implements action plans accordingly based on outcomes data.
+ Develops and implements the patient education process for patients with emphasis on cardiac rhythms, symptom recognition, risk factor modification and structured Care Coordination.
+ Screens all patients for heart rhythm disorder diagnoses and administers follow up phone calls to all post-procedure cardiac device implantation patients and addresses all problems that require attention to improve patient perception of care.
+ Assists the Cardiologists and core teams with identification, review and clinical outcomes.
+ Assists in data collection for research and registries for heart rhythm disorders.
+ Completes screening process for all potential patients for any type of cardiac device implantations.
+ Coordinates the scheduling of tests and procedures for patients and assists in communicating the results.
+ Coordinates and issues notes and orders for device patients including but not limited to admission and medications.
+ In conjunction with the Medical Director and the Cardiovascular Service Line, explores the capacity to market services to meet the needs of the patient population.
+ Develops, coordinates and participates in community awareness programs about cardiac rhythm, cardiac device implantation management annually.
+ Serves as a liaison between the patients, families and healthcare team throughout the process of diagnosis and treatment.
+ Compiles patient's current health record information, with investigation into current relevant test results, medications, Lab results, and health history. Preps charts prior to office visit, and shares with cardiologist to ensure best plan of care for patient.
+ Assists physician with pt office visits, navigating EMR, orders and functioning as a physician/ patient advocate during the visit.
+ Works to assist patients in coordination of the plan of care, including proper preparation of pts for tests and procedures, as well as follow up care.
+ Establishes, monitors, and revises educational materials for patient teaching. Provides individualized educational materials to patients and families for medications, diagnosis, tests/procedures and prevention strategies.
+ Triaging of patient phone calls, and patient communication of test results.
+ Assists providers with in-basket message clarifications.

+ Is an active member of hospital and community committees that are related to heart rhythm disorders and the care in cardiac device implants care and participates in community awareness programs.
+ Assists with the development of the annual budget for operating and capital expenses specific to the heart rhythm disorders programs. Ensures quality cardiac care is given while maximizing financial reimbursement and developing opportunities for cost savings.
+ Demonstrates motivation for learning through independent reading, professional networking and attending relevant conferences and seminars.
+ Acts as a resource, educator and consultant to staff. Provides patient and family education, support and follow-up as needed.
+ Provides outstanding service to all customers, fosters teamwork, and practices fiscal responsibility through improvement and innovation.
Qualifications
Minimum Education:
+ Bachelors Degree Required
+ Masters Degree Preferred
Work Experience:
+ 3 years Experience in nursing. Required and
+ 2 years Experience in electrophysiology acute care. Required
+ Previous supervisory experience. Preferred
Licenses:
+ Licensed Registered Nurse Upon Hire Required or
+ Registered Nurse Multi State License Upon Hire Required
Knowledge, Skills, and Abilities:
+ Excellent human relations and oral/written communication skills.
+ Demonstrated competence in program development and project management.
+ Analytical skills necessary to interpret quantitative and qualitative data.
+ Ability to direct and motivate individuals.
