Do you enjoy helping children and their families achieve their goals while working as part of a flexible and highly supportive team? Family Service Association has an opportunity as an Intensive Care Coordinator in our Community Service Agency Program. Hours for full time are 35 or 40 hours per week. The Intensive Care Coordinator (ICC) provides care coordination to children with mental health needs and their families. The ICC, while implementing the wraparound process facilitates the development of the Care Planning Team who utilizes multiple tools, including strength-based assessments inclusive of CANS in conjunction with a comprehensive assessment and other clinical information to organize and guide the development of an individual care plan and a risk management/safety plan to improve lives of families who have children with mental health needs.
The ideal candidate will have a willingness to work with youth and their families in their home and community.
The schedule is 40 hours weekly Monday through Friday 8:30 a.m. - 5:00 p.m. (This schedule is flexed to accommodate two evenings per week until 7pm).
Education and Experience:
The ideal applicant has obtained a MSW, LICSW or LCSW (or license eligible) with a Master's Degree in Social Work or Master's Degree in Counseling/Psychology or a related field with an LMHC or license eligible from an accredited school.-OR-
Have obtained a Bachelor's Degree in a human services field from an accredited academic institution and one year of relevant experience working with families or youth. If the bachelor's degree is not in a human services field, additional life or work experience may be considered in place of the human services degree.We offer a comprehensive benefits package including:
