We have an exciting opportunity for a Bachelor Level Intensive Care Coordinator to join our growing children's mental health program at CSA in Somerville. We are looking for a compassionate and culturally sensitive professional who thrives in a diverse and collaborative environment. We offer excellent supervision towards licensure as well! As a Bachelor Level Intensive Care Coordinator, your responsibilities will include: Provide comprehensive care coordination to children and families. Perform initial clinical assessments to identify family and youth's need and goals. Develop, implement and monitor individualized care plans based on family needs and goals. Facilitate care plan team meetings. Act as a liaison with collateral agencies and programs Advocate for families and educate them on self-advocacy Identify and assist youth and families in accessing necessary services Ensure proper documentation, and safety planning Participate in staff meetings, supervision, and training
Schedule: This position offers a hybrid schedule with flexible options. On-call availability during evenings and weekends on a rotating schedule is required to ensure continuous support for our clients.
Pay Rate:
$22.28/hour additional $0.96/hourly compensation for language skills
