Behavioral Health Assessment Specialist - Emergency Services - Full Time
Position Summary: Responsible for providing mental health services to patients presenting to the Corning Emergency Department or medical floors, including crisis/intake assessment and intervention, making appropriate referrals for acuity, providing education, discharge planning, acting as a community liaison, offering assistance to family members and insurance authorizations as needed. Education, License & Cert: LMSW Experience: Minimum of two years' experience working in a psychiatric setting or a crisis setting. Essential Functions: + Provide evaluations, assessments crisis intervention and referrals for all populations served in the emergency department and on medical floors. + Assist with substance use disorder patients and provide referrals for outpatient services. + Assist with resources and referrals for domestic violence, crime victims and families, homelessness. Provide assistance to case management to facilitate transition to outpatient services. + Demonstrates the ability to document understandable, comprehensible psychiatric evaluations and progress notes. + Responsible discharge planning and arranges follow-up care and referrals. + Functions as a liaison between the Behavioral Health services and community agencies and maintains knowledge of community resources that may be used in treatment planning. + Recognized and identifies actual or potential situations which places the organization at risk for corporate compliance violations. Communicates compliance issues/concerns with department manager and/or follows the policies for addressing the situation. + Mini mental state exams (MMSE) performed for patients whose mental capacity is questioned by the clinical team. + Provide emotional support for families and patients in acute crisis. + Works as a mandated reported to file with child protective services (CPS). + Assist with inpatient psychiatric care by working with psychiatric facilities across the state. + Facilitates completion of psychiatric paperwork in accordance to NYS Mental Hygiene Law. Other Duties: + Complies with department and organization policies and procedures. + Perform other duties as assigned.
