Behavioral Health Case Manager

**SUMMARY**
Provide Behavioral Non-Clinical Case Management and Administrative Care services for the Dental Readiness Classification (DRC) 3/4 soldiers who require behavioral health treatment.
**ESSENTIAL DUTIES & RESPONSIBILITIES**
+ Accept, process, scan, and file medical records related to the Soldier’s e-Case into the individual Soldiers’ charts. Track both temporary and permanent profiles and their expiration.
+ Track behavioral health status of all assigned Soldiers, including compliance and non-compliance.
+ Coordinate Soldier treatment plans with appropriate behavioral health facilities and/or social service agencies.
+ Provide all behavioral health/social service documents received for case management to Records Custodian to be filed using a document evidencing transfer of custody from the Contractor to the government.
+ Review their behavioral health cases at a minimum monthly and/or as needed and appropriate based on the needs of the Soldier and as directed by the State Behavioral Health Officer (SBHO).
+ Report on progress of all cases with new activity to the SBHO weekly during Behavioral Health Case Management meetings.
+ Work Special Medical Readiness Events (i.e., PHA/SRP events).
+ Maintain regular and punctual attendance.
+ Perform other duties as assigned.

**Job Requirements:**
**MINIMUM QUALIFICATIONS**
+ High School diploma or equivalent required. Associate degree in relevant area of study preferred.
+ At least one (1) year of relevant behavioral health background such as a mental health technician, crisis line worker, or social services case manager.
+ Must obtain and maintain any necessary security access and/or background checks.
**DESIRED QUALIFICATIONS & SKILLS**
+ Prior or current military Behavioral Health Technician experience preferred.
+ Ability to prioritize, organize, problem-solve, and coordinate daily activities to support effective improvement in individual patients.
+ Knowledge, skill, and ability to use basic office automation and information management equipment and programs (i.e., word processing, database management, email, etc.).
+ Excellent patient assessment skills.
+ Sufficient initiative, interpersonal relationship skills and social sensitivity. Ability to relate constructively to a variety of patients and healthcare staff from diverse backgrounds.
+ Ability to effectively read, write and speak English in order to communicate with all patients and other healthcare providers.
+ Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.
**WORK SCHEDULE**
Full-time. May be required to work additional hours as needed to complete assignment or project.

**WORK ENVIRONMENT**
Work is primarily performed in a professional office setting, including the normal office equipment (e.g., computer, fax machines, copier) and with a wide variety of people in differing functions, personalities, and abilities.
**PHYSICAL DEMANDS**
Position consists of sitting for long periods of time, using hands/fingers, handling, reaching with hands and arms, speaking, or translating. Periods of walking, standing, bending, kneeling, stooping, crouching, and lifting boxes weighing up to 30 pounds. Moderate to extended periods of office work with heavy periods of computer usage. **COMPENSATION & BENEFITS PACKAGE**
+ Pay starts $18.50 an hour.
