Earn $21.75/hour or $23.75/hour with the selection of Enhanced Pay, plus mileage reimbursement

Position Details

The Behavioral Health Case Manager provides community-based services for child and adult consumers in Bucks County. Focusing on rehabilitation and recovery, the case manager takes a consumer-driven, strength-based approach to case management and crisis services, while ensuring comprehensive and coordinated care.

Duties and Responsibilities

Act as the primary case manager for assigned caseload. Conduct bi-weekly support contacts at consumers' homes or appropriate community settings. Coordinate benefits, housing, and all other aspects of service delivery. Complete consumer-driven service and safety plans. Organize meetings with other services providers. Complete timely progress notes and productivity paperwork, ensuring paperwork is current. Collaborate with family and friends to locate and contact consumers.
