Position supports the Hospital Elder Life Program (HELP) a proactive delirium prevention program providing direct services to patients on the medical units of the hospital. The position works in conjunction with a team of volunteers, directed and trained by the position, to provide reorientation techniques, sleep hygiene education and care reminders for patients. The position operates both in direct clinical care and training/education.
PRINCIPAL ACCOUNTABILITIES: Provide bedside delirium interventions and protocols with medically admitted patients Compete chart review, program related records and metrics. Collaborate with various nursing and medical disciplines to provide delirium prevention and intervention care. Educate, orientate and coordinate hospital volunteers across the hospital units to provide delirium prevention interventions at the bedside. Engage patients with co-morbid medical and psychiatric presentations Provide clinical updates and progression reports to interdisciplinary teams. Provide education related to delirium, delirium prevention and interventions to the healthcare system and community. Maintain all documentation accurately and within the program designated requirements. Participates in departmental, cross departmental and system meetings and initiatives.
