Social Worker for Follow Up After Hospitalization for Mental Illness
CHA is engaged in a high-priority hospital improvement initiative to improve the rate of outpatient follow up after treatment for mental illness. Providing follow-up care to patients after psychiatric hospitalization can improve patient outcomes, decrease the likelihood of re-hospitalization and the overall cost of outpatient care. Yet individuals hospitalized for mental health disorders often do not receive adequate follow-up care. The Behavioral Health Urgent Care/Access Service provides short-term transitional care for patients transitioning to the community from acute levels of care. The individual hired for this position will provide brief, virtual, follow-up interventions following inpatient hospitalization, and facilitate patient engagement in outpatient care. They will also play a key role in the quality improvement initiative, and will collaborate with patients, their families, members of the multidisciplinary BHUC/Access team, and Inpatient Social Workers.
Qualifications:
+ Qualifications: Education/Training: Master's degree Social Work or Counseling
+ Licensure: Current MA licensure as LMHC, LICSW (Licensed Independent Clinical Social Worker), or LCSW (Licensed Certified Social Worker) considered with eligibility for licensure of LICSW within one year.
+ Expertise in engaging with patients dealing with serious mental illness, as well as motivational interviewing ,short-term and behaviorally-oriented therapies (e.g cognitive behavioral therapy, trauma focused-CBT, dialectical behavior therapy, parent management training) is preferred.
+ Must be credentialed by the CHA Medical Staff for practice in relevant clinical areas.
+ Bilingual in Portuguese, Spanish, or Haitian Creole is strongly preferred.
This is a fully remote position.
