Hennepin Healthcare is currently seeking a Licensed Mental Health Professional to join our Collaborative Care Model (CoCM) team as a Behavioral Health Clinician. This full-time role will primarily work day shift on-site on the Minneapolis downtown hospital campus. This is a grant funded position through a five-year grant from the Substance Abuse and Mental Health Services Administration (SAMHSA), in collaboration with the Minnesota Department of Human Services. Successful candidates must be a licensed Mental Health Professional per MN statute – LMFT, LICSW, or LPCC.
The Collaborative Care Model (CoCM) is a specific integrated behavioral health model, and is widely agreed upon as the integrated care approach with the most robust evidence base proving its effectiveness. Our team at Hennepin Healthcare is leading Minnesota in its implementation. It is a team-based approach to treating mental health conditions – especially commons ones like depression and anxiety – within primary care settings. The Behavioral Health Clinician (BHC) is a core member of the CoCM team, facilitating communication among all team members and acting as the lead contact person for the patient.
This position represents an expansion of our existing CoCM services, and applicants should feel comfortable working with a growing program and willing to share new ideas with leadership to help with continuous improvement efforts and long-term sustainability. The SAMHSA grant that funds this position is set to end in Fall 2028, but efforts are underway to continue this work into the future.

*SUMMARY*
The BHC is responsible for supporting and coordinating the behavioral health care of patients on an assigned patient caseload in partnership with the patient’s medical provider(PCP)and a consulting psychiatrist. Effective BHCs support the PCP in their role by coordinating the overall treatment plan, providing brief psychotherapy interventions, supporting medication management, alerting the PCP when the patient is not improving, and communicating with the psychiatric consultant regarding treatment changes. This role will also systematically track treatment response and monitor patients (in person and/ or virtually) for changes in clinical symptoms and treatment side effects or complications. Ideal candidates have experience in providing cognitive and behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, and solution focused brief therapy.
*RESPONSIBILITIES*
* Participates in multidisciplinary care team rounds
* Provides mental health services including initial and ongoing screening, diagnostic assessment, treatment planning (including regular updates), psychoeducation, psychotherapy services, crisis services, referrals, and discharge/termination planning
* Completes necessary documentation for accurate and timely billing of clinical services provided
* Provides psychoeducation to patients, families, and community organizations as needed
* Provides care coordination including discharge/termination planning
* Provides clinical supervision to graduate-level mental health practitioners in compliance with HHS and regulatory guidelines (e.g., licensing boards, state regulations, etc.)
* Assists in maintaining standards of accreditation by health management organizations and state licensure/certification
* Other duties as assigned

* Master’s degree in a behavioral health field

* Ability to read and interpret written instructions, patient charts, and physicians' notes
* Ability to interpret policies and procedures in an advisory capacity
* Ability to use computer systems, including electronic medical records, along with working knowledge of Microsoft Office Products (Outlook, etc.)
