psychosocial assessment and establishes a care plan as appropriateProvides direct patient care that may include case management care coordination interdisciplinary collaboration for care transition needs crisis interventionsafety planning counseling and emotional support to patients and families coping with illness trauma or end of life care referral and discharge planningRefers patient to appropriate levels of care and available resources that promote the individualized care planMaintains timely patient centered documentation thatreflect the patient and client systemsrsquo pertinent
