Summary :The Behavioral Health Collaboration Manager is a core member of the collaborative care team, including the patient’s primary care provider, psychiatric consultant, as well as the larger primary care team. This position will support and coordinate the mental and physical health care of patients on an assigned patient caseload with the patient’s primary care provider, and when appropriate to other mental health providers.

Job Description:

KEY ACCOUNTABILITIES:

Screens and assesses patients for common mental health and substance abuse disorders.  Facilitates follow-up care.Provides patient education about common mental health and substance abuse disorders and the available treatment options.Tracks treatment response and monitors patients for changes in clinical symptoms and treatment side effects or complications.   Analyzes data and reviews recommendations with the psychiatric consultant.Monitors psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.  Analyzes data and reviews recommendations with the psychiatric consultant.Provides brief behavioral interventions using evidence based techniques such as behavioral activation, problem solving treatment, motivational interviewing, or other treatments as appropriate.Provides or facilitates in-clinic or outside referrals to evidence based psychosocial treatments (e.g., problem-solving treatment or behavioral activation) as clinically indicated. Participates in regularly scheduled caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient’s primary care provider.  Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Tracks patient follow-up and clinical outcomes using a registry.  Documents in-person and telephone encounters in the registry and uses the system to identify and re-engage patients. Facilitates treatment plan changes for patients who are not improving as expected in consultation with the primary care provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.Facilitates referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment)Develops and completes relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.

QUALIFICATIONS:

Bachelor’s Degree in Nursing (BSN) or Master’s Degree in Social Work, or related behavioral health field.Registered Nurse (RN) or LCSW, LPC, or LMFTMinimum of 2 years working within health, with preference for experience in behavioral health.Experience with screening for common mental health and/or substance abuse disorders.Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders.Experience with assessment and treatment planning for common mental health and/or substance abuse disorders.

PHYSICAL DEMANDS:

Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance.Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties.

WORK ENVIRONMENT:

Climate controlled office setting with daily movement throughout the facilityInteraction with department members and other healthcare providers
