Join the MercyOne Family! We are looking to hire a Behavioral Health Coordinator.

As a Behavioral Health Coordinator at MercyOne, you will coordinate care in the Emergency setting to meet patient, family, staff, and provider needs to optimize resources utilization that connects the patient to appropriate and timely primary behavioral and community resources that are patient driven. Serves as a patient advocate in placement when resources are scarce.

Identifies patient's needs and determines appropriate level of care and proper referral.Evaluates potential admissions and/ or transfers for proper level of care; working with the ED staff.Completes all necessary documentation in a timely and accurate manner in compliance with standards for the State of Iowa.Assists with tele-psychiatry process in the Emergency department providing collaboration and patient support, a standard work flow, participation in daily huddles with the Emergency Department Team.Interacts with ER providers when tele-psych evaluation completed, and crisis stabilization plan is finished.Ensures that discharge patients have a follow-up appointment with a provider.Provides a follow-up phone call after an emergency department visit, within in 72 hours.Collects and reports weekly data, for Eastern Iowa Crisis Management System, and analyses.Works closely with field coordinator to provide continuity of care for behavioral health patients in the outpatient setting.Identifies and participates in quality improvement processes.Assists with regulatory readiness compliance in relation to Behavioral Health.Understands payor systems and impacts.

Schedule:

7:00 am – 3:30 pm Monday - Friday

General Requirements:

Active registration for Registered Nurse – Board of Nursing, State of Iowa.Must be BLS certified within 3 months of hire.Three years’ experience in health care delivery. Must be able to demonstrate basic computer skills in a Windows environment.Must work well with clients and families with diverse opinions, religious and cultural ideals and values. Must work autonomously with little direction.  Demonstrates ability to determine if client is in homeostasis regardless of living conditions or disease processes. Must possess leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, decision making, priority setting, work delegation, work organization, and customer service focus. Must be comfortable operating in a collaborative, shared leadership environment.Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.

Education:

Baccalaureate Degree in health-related field required or completion within three years of hire.
