I. Major Responsibilities:

1. Collaborates with health care team to identify alternative placements for patients.
2. Interfaces with members of the health care team, patients, families and vendors, while making referrals and transfer arrangements.
3. Extracts supplemental information from the medical record in order to make appropriate referrals to facilities.
4. Document’s progress of the discharge planning process in the medical record and in the MSM system.
5. Completes cases in the MSM after discharge.
6. Assists with completion of pertinent screenings and contacts appropriate regulatory agencies.
7. Establishes and maintains effective working relationships with facility representatives through telephone and/or direct contact.  Maintains a supply of all necessary forms related to the discharge process on inpatient units.
8. Facilitates referrals by telephone and/or fax to rehabilitation, skilled-nursing, or home care facilities.  Arranges for durable medical equipment (DME) as directed by care coordination team. 
9. Identifies and recommends solution regarding systems discharge issues.

7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

License/Certification/Education:
Required:
1. Bachelor’s degree in health services administration or related field.

Experience/Skills:
Required:
1. Excellent communication, collaboration, and organizational skills.  Knowledge of a variety of computer-based software programs such as Word, Excel, etc.


Preferred:
1. One to three years of experience in health care preferred. 

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed, and maintained in the individual departments.  The competencies will be maintained and attached to the departmental job description.  Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

Work is considered sedentary.  Position requires work indoors in a normal office environment.
