Your Responsibilities:

Conduct thorough assessments of patients' behavioral health needs, considering physical, psychosocial, and environmental factors.

Develop, implement, and evaluate individualized care plans to address patient needs.

Perform regular visits to monitor patient progress and adjust care plans accordingly.

Collaborate with physicians, interdisciplinary teams, and families to ensure comprehensive care coordination.

Provide education and support to patients and families regarding behavioral health care, symptom management, and overall well-being.

Supervise and guide behavioral health aides and LPNs in patient care delivery.

Identify and address changes in patients' conditions, reporting significant findings to appropriate team members.

Qualifications:

Active and unrestricted Registered Nurse (RN) license in the state of practice.

Minimum of one year of clinical nursing experience.

Experience in behavioral health, home health, or hospice care is preferred.

Ability to travel within the service area (up to 50%).

Reliable transportation, valid driver’s license, and current auto insurance as required by state law.

Associate or Bachelor’s degree in Nursing preferred.

Experience with Homecare HomeBase (HCHB) is a plus.
