Your Role as a Behavioral Health RN Case Manager
As a Behavioral Health RN Case Manager, you’ll be the heart of our patient care, advocating for their needs and ensuring they receive individualized, high-quality care. Your daily tasks include:
Case Management: Assess patients, manage a caseload, and provide holistic care.
Care Planning: Develop personalized care plans and adjust them as needed based on each patient’s evolving needs.
Collaboration: Partner with physicians and other healthcare professionals to provide seamless care.
Patient Education: Guide patients, families, and caregivers on managing symptoms and improving quality of life.
Team Leadership: Supervise and support behavioral health aides and LPNs to maintain exceptional care standards.

What You Need to Succeed
A valid, unrestricted RN license in your state
Minimum of 1 year of clinical experience in behavioral health, home health, or hospice care
Reliable transportation, a valid driver’s license, and auto insurance
Willingness to travel within your coverage area (up to 50%)
Bonus: Experience with Homecare HomeBase and a passion for behavioral health
