Job Description

Assist and monitor the Social Worker with on-site behavioral health screenings, assessments, consultations, and referrals for patients in a primary care medical setting. Educate and discuss short-term behavioral intervention and crisis intervention as needed under the direction of the Social Worker. Document appropriate health records in the electronic medical record. Follow up to determine the reliability and validity of treatments used and review notes from the provider on modifications to services and service delivery. Work collaboratively with staff and local human service organizations, treatment centers, and other community-based organizations to facilitate appropriate care coordination, information, and referrals.

Five days a week with hours dedicated to patient access (6-7 patients per day, 1-hour sessions). Eight hours allocated for administrative tasks. Office hours are Monday and Tuesday (8 AM - 7 PM), Wednesday to Friday (8 AM - 5 PM).

Pay and Benefits

The pay range for this position is $28.00 - $30.70
